Choose the General Dentist That’s Best for You
How It Works:
- Once you have joined our dental program. NO MEMBERSHIP CARDS WILL BE ISSUED. Instead your name will appear each month on your selected dentist’s roster verifying your coverage.
- Instructions: Please select ONE GENERAL DENTIST (not a specialist) from this list for your new application.
- To determine which dentist can best serve you, we suggest verifying with the prospective provider’s office the availability of appointments, ages seen in their office, cost, office hours, or any other criteria necessary to meet your unique needs. Feel free to visit their website or call their office number.
- Existing members who wish to change the dentist with whom they are currently assigned, should mail, fax (417-886-5852), or email 4wdw@att.net their request to the DENTAL SOURCE/AMERICAN DENTAL PROGRAM AGENCY office at the address listed below.
- Dentist changes will be effective on the first business day of the month if written requests are received in our local office before the 20 th of the preceding month.
- Specialist services may not be available in all areas and are not guaranteed in all areas.
